Once submitted, our Data Protection team will verify and process your deletion request within 30 days.
Important Notice for Healthcare Compliance:
Due to mandatory medical and healthcare regulatory requirements (under the Clinical Establishments Act and NMC guidelines), certain records regarding patient care, financial transaction histories, and audit logs must be retained securely for legal compliance purposes—even after your active user account is deleted.
If you prefer not to use this form, you can manually email support@oha.in with the subject line “Account Deletion Request”.
Please include your Registered Name, Phone Number, and Clinic Name in the email body.
