Important Notice for Healthcare Compliance:

Due to medical and healthcare regulatory requirements, certain health and medical records relating to your care may need to be retained securely for the minimum period required by law—even after your active user account is deleted. Any such retained data is used only for legal compliance and is not accessible for any other purpose.

Please include the app name (OHA for Patients), your Registered Name, and Phone Number in the email body.
